• If you do not have access to a printer, you will need to arrive 20-30 minutes early for your scheduled appointment to ensure there is enough time for you to complete the registration packet.

• If you arrive to your appointment on time and without a completed registration packet, we will have to reschedule the appointment to a later date.

• Children 17 and under will need to have a New Pediatric Patient Registration Packet filled out on their behalf.
